The service
Keep the ground you gained.
Periodontitis destroys the bone and tissue that support the teeth, and once treated it looks for a way back. The bacteria that cause it return; maintenance exists to stop them settling in again.
Each maintenance visit is active treatment, not just a polish. Plaque and tartar are cleared above and below the gumline, pocket depths are measured against your records, and anything moving the wrong way is caught while it is still small.
Between visits the home routine carries the load: brushing, flossing, and an antibacterial mouthwash. Kept up together, they protect more than the smile; periodontal disease is linked to heart disease, stroke, and other chronic illness.

Review
Your gums are checked against your last visit: pocket depths, bleeding points, and any recession.
Cleaning
Plaque and tartar are cleared above and below the gumline, the places a toothbrush cannot reach.
Assessment
X-rays when needed track the bone. If a site is relapsing, treatment is stepped up before it spreads.

How is this different from a regular cleaning?
A routine cleaning is prevention for healthy gums. Periodontal maintenance treats gums with a disease history: it works below the gumline and tracks pocket depths over time.
How often will I come in?
There is no single answer. The interval is set to how your gums respond, and adjusted at each visit.
How is periodontal disease diagnosed?
With a comprehensive exam: pocket depth measurements around each tooth, plus X-rays where the bone needs checking.
What are the warning signs of relapse?
Bleeding gums, persistent bad breath, swollen or tender gums, and teeth that feel loose or shifting. Any of these between visits is worth a call.
